Overview

The My Care Choices Register enables patients to share their wishes for future care, ensuring clinicians have access to preferences that matter most. This helps align clinical decisions with patient values and improves experiences across health and social care settings.

How to Use the Paper Version

The paper version can be downloaded from this website or provided directly by the GP practice upon request. Patients may complete the form independently at home or with support from a trusted person or healthcare professional.

Once completed, the paper form should be returned to the GP practice so it can be reviewed and uploaded to the MCCR (digital register). The My Care Choices paper form is intended to be used alongside the ReSPECT form; neither document replaces the other.

Both documents should be uploaded to the MCCR. Together, the information captured through the paper form and the ReSPECT form helps explore and record key preferences in more detail, ensuring the patient’s wishes are clearly documented, shared, and accessible to relevant professionals.

Frequently Asked Questions

How do I access My Care Choices Register?

MCCR requires the installation of Valida, as well as a signed DSA from your organisation. For more information on how to gain access please get in contact with a Regional Engagement and Support Lead.

MCCR uses the Valida client, which integrates with SystmOne and EMIS. It is also possible to access the MCCR using the web client, please get in touch for more information.

Similarly to accessing the MCCR using Valida, you must have a signed DSA in place, once this is done please ask your manager to get in touch with the MCCR team to arrange account authorisation.
