The My Care Choices Register holds information on your medical record around what matters to you most if you become more unwell or need additional care in the future.
A paper version of the information on the register is for you to keep as a reminder of some of the content on the register. The paper version is yours to keep in your yellow folder for those you consent to having access to it.
Make sure your care preferences are known to all services involved in your immediate care if you cannot express them yourself.
Support for loved ones, who will have clearer information about your wishes.
Improve communication with doctors, nurses, and care teams.
Ensure your values and priorities guide future care decisions.
The register is controlled by your health care professionals whilst the paper version is for you to keep at home. Information includes:
Medical conditions and how they affect you day to day.
Important things about you, your life, and what brings comfort.
Contact details of the people you trust to be involved in decisions.
What you would like to happen if you become more unwell.
What you would not want to happen (e.g., hospital admission or certain treatments)
If you support someone else, you may be there to help talk them through and complete the My Care Choices Register paper version. Once this is completed, you will be able to get their GP to enable their information to be entered onto the My Care Choices Register to be shared digitally with all the professionals involved in the care.
You MUST give a copy of the form to the GP so it can be recorded digitally.

Keep a copy in an easy-to-find place
Tell close family or carers where it is
Ensure everyone involved in their care is aware of the My Care Choices Register
