When the person does not show on our records I will always check to see if they are listed on the My Care Choices Register and sometimes the information shown here can provide invaluable information to guide the ambulance crew on what to do next.”

And he’d spoken to our St Helena community nurse about the My Care Choices Register. All of that was in place. It made it a bit easier because those difficult conversations had already been had when he was well enough to talk about it.”

“One of the people I support is a young lady who has not been ready at all to discuss her future planning. When I saw her a few weeks ago, I tried talking to her about the MCCR and she really didn’t want to go there. However, I mentioned we had a new booklet and that it was something I could leave with her to look at in her own time and to complete if she felt that she could.
At my next visit I gave I her a copy of the My Care Choices Record and we looked at it together. She was impressed at the way it was laid out, said it was simple to understand and instead of having to talk about her options it would be easier this way because she can write her choices down.”
